Consent and Agreement

Indemnity and Liability information to be shared within your Club when entering the league. It is expected that all managers and club owners will ensure that their players and staff will be made aware of the indemnity information below. The information below will be listed on the player registration link for all boys and girls together with the parents. It is important for all managers and club owner to READ the following information and click "Yes" as a sign of acknowledgement and understanding in order to continue your request for registration of your Club for the 2024 Season.
PERSONAL DATA PROTECTION: I understand and agree that in order to participate in all or any of the above- mentioned events I am required to supply certain information about myself and my child in this form and in any related documentation. I understand, agree and consent to Eagles Basketball collecting personal information relating to my child and that this information will be used and stored by Eagles Basketball for the purposes of administering and for the specific purposes described in this form. I declare, further, that to the best of my knowledge, my child is of good health and physically able to attend all or any of the above-mentioned sessions, matches and tournaments.
PERMISSION TO BE FILMED & USE OF LIKENESS IN ALL MEDIA: I acknowledge that Eagles Basketball may be filming and/or photographing activities for promotional, advertising, or other commercial purposes and hereby consent and agree that Eagles Basketball and/or its agents may do so. I hereby irrevocably grant Eagles Basketball (and its affiliated companies and agents) the right in perpetuity and throughout the world, without any expectation of compensation, to use such material, as well as the portrayal of my name, image, likeness, and voice, in ANY media and on any printed material, in connection therewith for the purposes of promoting or advertising Eagles Basketball Club activities.
DURATION: I understand and accept that this indemnity shall be enforceable for so long as my club is in the league. I consent that it shall not be necessary for any further indemnities to be signed by my club during the period that my club is in the league and that this indemnity shall be sufficient and binding on both players and staff.
ALL CLUB MEMBERS SHOULD SIGN OR ACCEPT THE FOLLOWING DURING PLAYER REGISTRATION IN THE LEAGUE: I/we, the parent/s on behalf of our Child/ren, allow him/her to participate at his/her own free will in any or all matches / cup competitions / Area trials as well as festivals being run by Eagles Basketball and its partners, (“the Events”). ASSUMPTION OF RISK: I acknowledge that my child’s participation in the above leagues and tournaments may involve some physical risks of injury, severe bodily harm or fatal incidents, given the physical nature of the sport of basketball, and we, the parents, freely and voluntarily choose to accept these risks on behalf of our child. INDEMNITY: I hereby irrevocably and unconditionally indemnify and hold harmless Eagles Basketball, its officers, directors, employees, affiliated companies, coaches, officials and agents, against any and all claims, liabilities, losses, injuries, theft, costs, expenses (including reasonable attorneys’ fees) and damages, whether direct, indirect, consequential or otherwise, arising from any cause whatsoever from our child’s participation in any way howsoever in the Leagues and Tournaments and which I, the child, their families and/or dependents may suffer and/or incur. INCAPACITY & MEDICAL TREATMENT: In the event that my child is incapacitated, I hereby grant permission to the leader/organizer of training sessions, camps, leagues and tournaments to render or arrange for immediate medical or other treatment as may be immediately necessary and agree that I shall be fully responsible for any costs arising out of such medical or other treatment.

Clubs are required to register their teams before the start of the 2025 Season. Parents & Guardians of players will not be able to register the players for the 2025 season without the Club information appearing on the platform. Registration Deadline is 15 February 2025.
